PRIVACY STATEMENT
The Office of Rural and Northern Health (ORNH) has created this privacy statement in order to demonstrate a firm commitment to privacy. The following discloses the information gathering and dissemination practices for the ORNH Personal Information Form and web site: www.ornh.mb.ca.
Personal Information Form
Contact information gathered from the ORNH Personal Information Form is used solely for the creation of interdepartmental statistics and maintaining on-going contact list between the registrant and the Office of Rural and Northern Health. All information is kept confidential and is never sold or given to third parties.
Email addresses are used to forward notices of relevant upcoming events, and newsletters to those who have requested receipt of such documentation.
The ORNH Personal Information Form requests that users provide contact personal and demographic information, educational information and employment information. The form is available online and offline as a handout. We provide the informant the option to send this information by mail or fax.
The method of submission is as follows:

Faxes and mail are received in the Administrative Office of the Office of Rural and Northern Health and are kept confidential.
Signature is required as authorization to keep and gather information. If registrant is a minor (under the age of 18) the form must be authorized by a parent or guardian.
